Support that grows with each person.
From the first assessment onward, everything we do is shaped by each resident’s strengths, needs, and goals.
Our program
Assessment
Each resident receives an initial functional assessment that evaluates their strengths, challenges, and needs.
Individualized Service Planning
Following the assessment, each resident helps develop their own service plan with support from program staff.
Case Management
We make sure each resident’s physical, social, behavioral, educational, vocational, and psychological needs and preferences are identified and addressed through service coordination and regular feedback.
Life & Social Skills Training
Residents receive daily individual and group instruction toward the skills outlined in their service plan.
Recreation
Residents are exposed to a wide array of recreational activities chosen to build interests, knowledge, and social skills.
Sensory & Mental Stimulation
Activities designed to engage all five senses and keep minds active.
Family Involvement
Weekly activities encourage regular visits, shared fun, and consistent communication between staff, families, and authorized representatives.
Horticulture
Hands-on sessions in caring for plants, fruits, and vegetables — how to plant and harvest food and help keep a healthy environment.
